You will work collaboratively with the Aging and Disability Resource Center (ADRCs), Area Agencies on Aging (AAAs), and Centers for Independent Living (CILs) and the State Units on Aging (SUAs), as required to meet Veterans needs for program participation. Guide community providers in the Veteran Care Agreement (VCA) process. Develop agreements with the local Aging and Disability Network Agencies (ADNAs) to assure participation criteria and VCA agreements are in place and designate them as providers for VDC services. Authorize VDC services appropriately and responsibly. Review and approve spending plans, monthly spending reports, and requests for changes in budget or spending plan. Reimburse provider in a timely and accurate manner. Work closely with Person-Centered Counselors to ensure assessments and services are completed in a timely manner. Work closely with Authorized representatives to ensure services and spend plans are executed appropriately. Accepts referrals for the VDC program and process consults according to consult guidelines. Review the spending plan and all supporting documents monthly to assure no duplication of services. Approve or propose changes to the spending plan according to Veteran's needs. Meet as needed with Person-Centered Counselor and community provider as needs to reassess and modify plans according to Veteran's needs. Review and approve any new purchases of goods or services as appropriate. Address any Veteran, provider or person-center counselors concerns or complaints within 48 hours. Provide nursing oversight of Home Maker, Home Health, Home infusion, Hospice, Caregiver Support and Respite Care Programs. Provide assistance in referrals to community based long term care including home health and home hospice. Provide oversight of care provided by community agencies on behalf of the VA. Understand and follow policies and procedures related to contract placements in through community care for both Purchased Care Services (PCS) and through the Program of Comprehensive Assistance for Family Caregivers program (PCAFC). Utilize electronic verification and processing systems such as the Consult Toolbox (CTB), Health Share Referral Manager (HSRM), Provider Profile Management System (PPMS) and the TriWest portal, in order to complete authorizations for care. Provide patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. Demonstrate advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients. Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient care problems.

The VA Texas Valley Coastal Bend Health Care System is an ambulatory health care system with services in medicine, surgery, psychiatry, and rehabilitation medicine. We operate a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program, which include the Corpus Christi Outpatient Clinic, Corpus Christi Specialty Clinic, Harlingen Outpatient Clinic, Health Care Center at Harlingen, Laredo Outpatient Clinic, and McAllen Outpatient Clinic.
Through utilization of inpatient/emergency department contracts, we manage approximately 2600 inpatient admission and 5800 ER visits annually. Affiliated with the University of Texas Rio Grande Valley Medical School, we have an ambulatory care program serving as a teaching/training site for medical residents.
